Fee Schedule and Terms of Service

The Juneau Family Health and Birth Center (JFHBC) is a non-profit organization supported in part by member community donations. Each client will be responsible for some or all of the fees pertain their care. We are happy to bill Denali Kid Care and/or your company.  We ask that you pay any costs you are responsible for in accordance with the payment plan you have negotiated and signed below.

What Will Our Services Cost?

This may vary depending upon your individual circumstances. We will work with you to provide an estimate of your portion of the costs. Because this is just an estimate, you will receive a monthly bill with your specific charges or a reimbursement at the end of your care depending on these circumstances.

How Does Denali Kid Care/Medicaid Work?

For moms with Denali Kid Care/Medicaid (DKC), all costs are reimbursed by DKC or donated by JFBC except, massages, supplements and supplies. You are responsible for paying those costs directly. Payment is expected for massages, supplements and supplies at the time you receive those services or items. We have a sliding scale based on income available to all.

How Much Will My Insurance Reimburse?

Each insurance plan is different. Most have a deductible and a co-payment amount. If you are unsure about what your insurance will pay, you may call them directly. Please have this information when you come for your initial prenatal exam. Payment of your co-payment, deductible, and any services and supplies which your insurance will not reimburse is expected at the time the service is rendered or the estimated portion of the OB Package is to be paid before your due date.

What if I Don’t Have Any Insurance or DKC/Medicaid or Medicare?

We provide services to many families who self pay. Please talk with your midwife, our front desk staff or our billing person to complete a payment plan. We have a sliding scale for the Midwife OB package based on income available for all.

Are There Other Costs?

Physician visits at JFHBC or extraordinary diagnostic tests (such as ultrasound) and physician visits outside of JFHBC are your responsibility and will be paid to the clinic, hospital or physician involved. Also, in the event of a transport to the hospital, any service rendered by the ambulance or hospital is your responsibility.

Transfer of Care Policy

In the event that you transfer to another care provider before labor begins, you will be charged only for services you have received. If transport to the hospital is necessary after onset of labor, we will charge for labor monitoring by the hour. We will remain with you until after the birth and continue to provide postpartum care as needed until six weeks postpartum.

    In addition, we offer a sliding scale based on income upon request.

    We will not turn anyone away for financial reasons, but we expect you to take responsibility for making arrangements with us.
